Youth Registration

Please complete both Camp registration and the Permission and Medical Release forms below.  These forms are for Camp Ritchie and for Grad Camp.

Please print out the Church's Permission and Medical Release form below, fill out with YW name and parent signature only and turn into your ward camp specialist. (There's no need to fill out the rest of the form as that information is contained in the camp registration form.)

If your camper needs to bring prescription medication(s) to camp, please download this form and follow the instructions.
